What country & city was Avicenna from? What was he?

Avicenna lived his entire life in Persia (= modern Iran). Born around 980 CE, he died in CE 1037 in Isfahan. He wrote on many subjects--law, philosophy, medicine. Best known to me as the author of Kitab al-Qanun, translated into Latin in the twelfth century perhaps by Gerard of Cremona and known to Europe as the Canon. In it he summarizes and organizes the medical theory and practice of Galen, Dioscorides, and others into a 5-book coherent whole. He wrote in Arabic, the lingua franca for science and medicine in the near east.
