Why is James' brother upset when he picks him up from the clinic?

The first thing James does after leaving the clinic is to ask his brother to take him to a bar, something that upsets Bob greatly. When they arrive James orders a pint of whiskey, examines it, smells it, and asks the bartender to pour it down the drain.
