How did Casey's mother die? When did she die?

Casey's mother died in the past year, around six months ago as the novel begins. She was living in Phoenix but had gone on a trip with friends to Chile, where she unexpectedly died of an unknown cause. Her death preoccupied Casey throughout the novel and she never understands what happened.
