What does each of Julia's scratchings on her watch represent?

There are three different types of scratches on Julia's watch. Every full moon represents one of Julia's patients who has died. The little crescent moons are the babies who died. The little scratches are those who had been carried long enough that one could tell whether it was a girl or boy but were stillborn or miscarried.
