How did William Wharton get the opportunity to try to strangle Dean Stanton?

From the time the guards picked Wharton up at the hospital and throughout the ride to the prison, Wharton acted drugged and docile. When Wharton got out of the vehicle with three guards, he suddenly took his chained arms and put them around Dean Stanton’s neck and tried to strangle him to death.
