What is the main conflict in The Cider House Rules by John Irving?

The main conflict in this novel occurs between the two protagonists. Dr. Larch is pro-choice; Homer is pro-life. Dr. Larch believes that women should have the choice to end a pregnancy in a safe manner; Homer, an orphan, believes that life is our greatest gift.... a gift that shouldn't be taken away.
