What does Richard see while in the Valley of the Lost, what happens to Verna, how does Richard deal with it and what is her reaction?

Richard sees a vision while they are in the Valley of Kahlan being beheaded. Sister Verna is trapped in a spell and Richard frees her. After the two of them are safe, Sister Verna tells Richard how angry she is with him but does not fully explain why.
