What does Tyler do and how does he feel when men burn down Ina's clinic?

Tyler watches from the rice field as the men burn down Ina's clinic. He waits until they depart and falls asleep. After waking up, he meets again with Ina who thanks him for saving En's life. He feels very weak, and then loses consciousness. He wakes up days later in an abandoned house.
