What evidence does give to explain the current state of the planet during Jimmy's time

The planet is full of disease-ridden pleeblands. The only places free of disease and poverty are the vast opulent lands of the Pharma companies that are cut off from the rest of the world. Disease is so ubiquitous in the latter that residents who do not normally reside in the pleeblands either avoid entering them or take special medicines to enter them safely.
