What happens to Johnny when he tries to leave his apartment to visit Tatiana?

When Johnny even reaches for the doorknob, he begins to have excruciating pain in his chest. On the outside he hallucinates about being hit head-on by an oncoming trunk. Of course, he is fine and travels to Tatiana's without further incident.
