Why is Scarpetta unable to establish a cause of death for Fred Cheney in the novel, All That Remains?

It appears on autopsy that Fred suffered some soft tissue injury, such as a cut throat, that resulted in his death. However, Scarpetta cannot be sure what killed Fred because there is no indication of violence on his bones, the only parts of his body still in existence.
